Asthma Disease Management Program
Our Asthma Clinic offers care on both an inpatient and outpatient basis to help you maintain a stable state of health regarding you or your child’s asthma. Certified Asthma Educators will help you learn about your disease and design action plans for maintenance and relapse so you can manage your asthma in your everyday life.
How We Treat
When you or your child attends our clinic, you will work with Certified Asthma Educators to learn how to manage your asthma. The outpatient asthma program helps to reduce risk and identify any triggers through education about the disease, wellness and medication. If you or your child requires acute hospitalization for any issues regarding your asthma, you may be directly admitted to Hospital for Special Care from a hospital emergency room, or from a physician’s office for symptom management. Our staff will help coordinate the action plans we design with your or your child’s primary care physician (PCP) and/or school nurse.
We will help you fully understand you or your child’s disease, identifying any triggers and making action plans so that you or your child are able to best manage your asthma in everyday life and prevent relapse.
We work with you and you or your child’s PCP and/or school nurse to manage your medications appropriately.
