Advance Practice Nurse (APRN)
(Full-time, 6:00 pm – 7:00 am; E/O weekend, Department of Medicine)
Required: Master's Degree in Nursing (MSN); minimum of five years of clinical experience with demonstrated leadership and teaching skills; current licensure as an Advanced Practice Registered Nurse in the State of Connecticut; must have passed the State of Connecticut Pharmacology course; possess DEA licensure for narcotic prescription; basic and life support certification and advanced cardiac life support (ACLS). On the evening, night, weekend, and holiday shifts the APRN will be responsible for promoting excellence in patient care by serving as a clinical role model, physician extender, educator, and consultant. Assess designated patients and family care needs, and collaborate with the treatment team to ensure high quality patient care.
Behavioral Tech
Neurobehavioral Program
(Part-time, Days and Evenings, E/O WE)
Required: Bachelor’s Degree in Psychology, Sociology, or healthcare related field; valid current driver’s license; ability to read, write and understand instructions. Six months experience working in a rehabilitation/treatment or human services setting and experience working with groups or teams is preferred. The Behavioral Tech is a paraprofessional care giver who assists in the care and treatment of patients who have sustained an acquired brain injury. The Behavioral Tech works/functions under the direction and supervision of the Team Leader and/or Registered Nurse.
Clinical Nurse Leader
(Full-time, Days, occasional W/E, Inpatient Rehabilitation Unit)
Required: Master’s Degree in Nursing; Graduate of an accredited school of nursing; current licensure as a registered nurse in the State of Connecticut; minimum 3 years recent, progressive clinical experience in a setting relevant to the population served; demonstrated leadership, communication, and critical thinking skills; experience with quality improvement and evaluation of performance metrics; demonstrated professional engagement and commitment to lifelong learning; ability to collaborate effectively with an interdisciplinary team. Graduate (or currently enrolled in) a Clinical Nurse Leader program and certification as a Clinical Nurse Leader from the American Association of Colleges of Nursing are preferred. The Clinical Leader is a master’s prepared nurse generalist who provides lateral integration of care services within a microsystem of care using evidence-based practice to effect quality, safety, and patient care outcomes. The CNL will be responsible for the ongoing coordination and oversight of patient care activities for a designated cohort of patients. The CNL designs, implements, evaluates patient care by coordinating the activities of the nursing team in collaboration with other disciplines to achieve identified patient care outcomes.
Registered Nurse Clinical Coordinator – Hartford Satellite
(40 hours; days; weekends as needed)
Required: Graduate of an accredited school of nursing; current CT RN license; minimum 5 years clinical experience in related setting; demonstrated competence in clinical setting; demonstrated ability to collaborate with an interdisciplinary team, critical thinking, and leadership skills. BSN and current CPR/BLS Certification are preferred. The RN Clinical Coordinator is a front line leader responsible for the ongoing coordination and oversight of patient care activities for a designated clinical unit or population of patients. The Clinical Coordinator assures functions of assessments, monitoring, and evaluating outcomes of patient care. The principle domains of the role are patient care, leadership, care management, and education. Utilization review, case management, discharge planning and quality initiatives are components of principle domains. The Clinical Coordinator is directly responsible for facilitating patient care by coordinating the activities of the nursing team and collaborating with other disciplines.
