When faced with rehabilitation needs resulting from a complex medical problem, our inpatients find programs and services designed to help them regain independence. Our multidisciplinary team implements a specialized program for each patient designed to optimize independence after a complex injury or medical problem. HSC’s approach to medical care and rehabilitation begins with and finds success through teamwork. Physicians, therapists, nurses, social workers, case managers, administrators and other healthcare professionals work directly with patients and families to set and achieve their individualized goals—with one ultimate patient goal in mind: going home. At HSC, our team of highly-skilled physical, occupational, speech and recreation therapists work together to help patients achieve their maximum level of functioning and independence. At HSC our professionals work hard to ensure the highest level quality care. The Commission on Accreditation of Rehabilitation Facilities (CARF) has given HSC a prestigious three-year accreditation in the areas of Brain Injury Rehabilitation, Inpatient Rehabilitation, Outpatient Medical Rehabilitation, and Spinal Cord System of Care.

Patients or families are always welcome to visit and tour HSC. Upon referral to our facility, the patients’ insurance company is notified and coverage is verified. HSC’s highly-respected medical staff is composed of dedicated specialists in internal medicine, infectious disease, geriatrics, pulmonology, neuromuscular medicine, neuropsychology, wound care and pediatrics. Our in-house physiatrists are experts in the physical rehabilitation of patients with spinal cord injuries, stroke and acquired-brain injuries, as well as with care of medically-complex patients who require rehabilitation. HSC’s on-site resources include a fully-staffed laboratory, pharmacy and radiology department that provides general diagnostic and ultrasound procedures. The hospital maintains close ties to a network of local consulting physicians who represent a full range of medical specialties. From admission to discharge, each stage of care is planned and delivered by an interdisciplinary team of talented healthcare professionals — all of whom share HSC’s commitment to rebuilding lives.
At HSC, our continuum of care focuses on meeting the health care needs of adults, adolescents and children. We specialize in serving patients and community members with a wide range of medical complications. Our continuum of care encompasses a variety of programs and services designed to meet individuals health care needs over a span of time. Patients enter this continuum at different levels. For instance, patients may be admitted to our Close Observation Unit (COU)  to wean from a respirator or ventilator and while on that unit they may receive therapy services if appropriate. Once successfully weaned, patients may then transition to our Inpatient Rehabilitation Unit  for more intensive therapy services and medical management. They will work on goals they developed with the health care team. Once their goals are met patients are discharged home, they may choose to transition to our Outpatient Rehabilitation  facility to receive further therapy services. Upon completion of outpatient therapy, many individuals join the HSC Aquatic Rehabilitation and Fitness Center  to maintain their strength and endurance.
