This Cardiac Care Initiative consists of treatment, therapy and education for patients challenged by heart failure. The goal of this program is to stabilize a patient’s medical condition, while improving his/her functional ability, which will afford the patient an opportunity to return home and to community quicker.
The following is a partial list of patient candidates who may benefit from this program:
- Patients who have received a diagnosis of heart failure, were discharged from an acute-care hospital within the last 30 days, and require a period of treatment and strengthening to enable them to return home and to their community;
- Patients who have had recurrent admissions to an acute-care hospital for management of their heart failure;
- Patients who are preparing for the implantation of a cardiac mechanical assistive device such as a left ventricular assist device (LVAD), and need strengthening prior to surgery,
- Patients who have recently received a LVAD, and are coming to HSC for treatment, therapy and education prior to returning home.
