Acquired Brain Injury, Neurobehavioral Program Admission and Discharge

Admission Criteria

(must meet all criteria) Patient must:

• Have a diagnosis of an Acquired Brain Injury (ABI)
• Be medically stable
• Be 18 years or older
• Have maladaptive behavior requiring a secured unit
• Demonstrate the potential to improve functional ability
• Have a conservator
• Not have a degenerative neurological diagnosis
• Have a viable discharge option 

Discharge Criteria

The length-of-stay in the Neurobehavioral Program is based on each patient's progress in program and the NBP team's assessment of his/her readiness to rejoin the community in a less restrictive environment. To be discharged:

• the patient has demonstrated progress to a level that a secured unit is no longer required, or
• the patient's progress has reached a plateau, or
• the patient develops an acute medical or psychological condition requiring a different level of care.

Discharge planning is initiated during each patient's preadmission process and continues throughout his/her stay. The discharge planning process includes assisting patients, conservators, and/or families in identifying viable community support services for the patient.
