Admissions Process

A patient is usually referred to Hospital for Special Care (HSC) from an acute care hospital. HSC Admitting Nurse Evaluators receive referrals from patients, families, physicians, case managers, and social workers. Once a referral is received, the Admitting Nurse Evaluator will request and review medical information, and may conduct an on-site evaluation in order to determine if the patient is clinically appropriate for admission to one of our hospital's programs. You may also contact the Case Manager at the hospital where the patient currently is admitted to request a referral to HSC.

Evaluating HSC

Family members/significant others are invited to come to HSC to see the facility and meet with staff to discuss services and the admitting process.

To ensure that a staff member is available to spend an adequate amount of time to conduct a tour and answer questions, we ask families to call to schedule an appointment. Please call the Admitting Office at 860.827.4715 or toll-free 1.800.220.7723.

Insurance Verification

Prior to admission, a staff member will work with the patient's family to verify health insurance benefits and information. The patient and family can then consider the financial impact of the anticipated admission to HSC.

Admission Day

Upon admission to HSC, the patient or designee will be interviewed by an Admissions Coordinator to ensure that patient, family contact, and insurance information are correct.

Please present these documents upon admission:

  • Insurance cards (private insurance, Medicare, Medicaid)
  • Photo identification card/driver's license
  • Living will, power of attorney durable power of attorney, health care proxy, conservator papers
  • Copy of organ donation request card
