Hospital for Special Care conducts a periodic, comprehensive Community Health Needs Assessment (CHNA) to evaluate the health needs of individuals living in the hospital service area of the state of Connecticut. The purpose of the assessment is to gather information about the health needs of community residents. The CHNA helps guide HSC in developing and implementing strategies to improve the health status of those residents.
