Request Medical Records

Patients treated at Hospital for Special Care may request a copy of their medical records by sending a written request to the Health Information Management department:

Please specify the type of information and/or dates of service that you want to be sent from the medical record.

Include:
  • Patient’s Full Name (including maiden name or other names that may be needed to locate the correct record)
  • Date of Birth
  • Dates of Service
  • Patient’s Current Address
  • Telephone Number where you can be reached with questions
  • Signature, Date and Printed Name of the patient or other authorized person making the request

Fees

There will be no charge for the following:
  • If copies are going directly to another health care provider.
  • Copies released to a patient's attorney or authorized representative if the record is needed for supporting a claim or appeal under any provision of the Social Security Act or veterans’ benefits, and the request is accompanied by documentation of the claim or appeal.
  • Copies to support Worker’s Compensation cases or other similar programs under state law for work-related injuries or illness.
  • No charge for copies of records you request for yourself, or in the case of an authorized person (conservator) will not be charged for personal copies
  • No charge for patient directed requests to another person

For copies of medical records a reasonable fee may be charged.

If a fee is charged, you may receive a bill from CIOX Health. (CIOX Health is a national vendor that processes requests for records on behalf of the Hospital.)
